Deaths from Chronic Diseases
Herefordshire Primary Care Trust is seeking to reduce the number of deaths from the following chronic conditions: diabetes, chronic respiratory disease, asthma, coronary heart disease and stroke.
During 2002-04, Herefordshire had 180 deaths from chronic conditions per 100,000 population, this figure decreased over the following years to 169.60 in 03-05 and 150 in 04-06.

This indicator formed part of Herefordshire's 2006-09 Local Area
Agreement (for more information about the
LAA see Reference
Section). Using 02-04 as the baseline (180.4 deaths per 100,000
population) the target for 2005-07 was to have fewer than 174.7
deaths from chronic conditions. 2004-06 figures are already well
below this target.
