Herefordshire Community Strategy is delivered through organisations, networks, sectors and groups working together to co-ordinate activity and avoid duplication. This happens through Herefordshire Partnership. A single three-year Action Plan is published and updated each financial year, which includes:

• Outcomes
• Performance Indicators
• Actions
• Timescales
• Progress Reports
